One paper copy and an electronic version of the report should be submitted to the HR Project Officer (HR Department, Level 1, 88 Musk Avenue, Kelvin Grove; hrprojectofficer@qut.edu.au) within one month of the staff member's return from the program.

The staff member's name, faculty/division, report title and the dates during which the leave was taken should also be at the top / on the front cover of the report.

The report, which should not exceed five (5) pages, is to include the following headings and information:

1. Abstract

Include a 150 to 300 word abstract which summarises the principle outcomes and benefits of the program to the staff member and QUT.

2. Outline of the program

Include the original learning objectives approved for the program. If a major change was made to the original program this should be indicated together with reasons for the change.

3. Objectives and benefits

Indicate how learning objectives were achieved and evaluated. Include additional benefits of the program. Detail any obstacles encountered in meeting these goals.

4. Dissemination of Information

Indicate how the knowledge obtained on the Professional Development Program was disseminated to interested bodies and staff members. Included in the report must be a statement from the staff member's immediate supervisor noting and approving the staff member's dissemination of information gained during the period of Professional Development Program leave. The signed original of this document should be included with the paper copy submitted. Unsigned copies are acceptable with the electronic version.

This should include any recommendations for further action which may benefit the University.

8. Financial statement of external income received during the program

The report must include a financial statement of all sums of money received by virtue of the staff member's leave from sources other than the University. This statement must be included, even if the amount is nil.
