The following template has been developed by the QUT Faculty of Science staff who have kindly agreed for the template to be placed on the Health and Safety Advisory Services web page for QUT staff to use and change as needed.
How to complete the template
The boxes on this template should be filled in after checking with relevant staff who are in Health and Safety roles to ensure currency, and management approval to be in these roles. Copies of relevant and current certificates of qualifications should be kept at the local workplace.
- The Warden information section on the template should be completed after checking with the Building Warden for the building or refer to the following spread sheet which is maintained by Campus Services.

- The Workplace Health and Safety Officer/Workplace Health and Safety Representative section should be completed after checking that staff are current and have been nominated by management for the role of WHSO or elected by staff as the WHSR.
- The first aid Officers section requires the names of the first aid officers who are designated and voluntary approved first aid officers for the floor/ level, or Section or building. Staff must be current in their qualifications as first aid officers.
- The additional information section can include where the first aid kit/s are located, the location of the Safety Data Sheets for the floor/level, or the location of a Spill kit, or the name of the Practising Area Radiation Safety Officer PARSO if these are appropriate for the workplace.
- The template should have a version number and date and review date included on it.
