Introduction

Reporting all safety problems, incidents and accidents in your area is important for the safety and wellbeing of everyone at QUT. You can significantly improve the ability of your local Health and Safety Committee to identify and address hazards simply by reporting all safety problems, incidents and accidents.

The Safety Problem Report and Investigation Form (SPRI) has been developed to satisfy legal reporting requirements and to capture the information which your Health and Safety Committee can use to best plan risk management strategies in your work area.

Please Note: If there was a serious bodily injury, dangerous event, serious electrical incident or dangerous electrical event, the QUT Human Resources Department- Health & Safety Advisory Services must be notified A.S.A.P – 3138 9269.
