Introduction
Reporting all safety problems, incidents and accidents in your area is important for the safety and wellbeing of everyone at QUT. You can significantly improve the ability of your local Health and Safety Committee to identify and address hazards simply by reporting all safety problems, incidents and accidents.
The Safety Problem Report and Investigation Form (SPRI) has been developed to satisfy legal reporting requirements and to capture the information which your Health and Safety Committee can use to best plan risk management strategies in your work area.
Please Note: If there was a serious bodily injury, dangerous event, serious electrical incident or dangerous electrical event, the QUT Human Resources Department- Health & Safety Advisory Services must be notified A.S.A.P – 3138 9269. For definitions see page 4 of the Safety Problem Report and Investigation Form.
Filling in the form?
For an incident or an accident:
- The person involved or another person who was present should complete pages 1 & 2 of the four page form.
- The Safety Problem Report and Investigation form should be filled in within 3 days of the incident occurring and a record kept for 12 months.
- Page 3 is completed by your local Workplace Health and Safety Officer (WHSO) or Workplace Health and Safety Representative (WHSR) or Supervisor to further investigate the safety problem or accident at the workplace, if appropriate. The investigation is done prior to the SPRI form being sent to Health and Safety Advisory Services.
- Page 3 of the SPRI form must be signed by the Manager/Head of School before forwarding to Health and Safety Advisory Services
- The original SPRI form should be sent to Health and Safety Advisory Services and the copy kept at the local workplace. The type of accident or incident or hazard and the control measures on the SPRI copy, can be shared at the local Health and Safety Committee.
For a safety problem (if no incident, accident or injury has occurred):
- Simply complete the personal information section and the safety problem section on page 1. Make sure that the location of the problem can be identified.
- Inform your Workplace Health and Safety Officer or Workplace Health and Safety Representative or Supervisor of the safety problem and that you have completed the SPRI form.
What do I do with the form once it is filled in?
- Check to see that pages 1 and 2 have be filled in, including the date the form was completed.
- Has page 3 been filled in by the WHSO, or the WHSR or Supervisor? Has page 3 been signed by the Manager/ Head of School? If the form is complete, copy the form, give the completed copy to your Workplace Health and Safety Officer or Representative for the Health and Safety Committee.
