Introduction
A risk assessment approach to the management of health and safety is one of the key elements of the Queensland Workplace Health & Safety Act 1995.
To assist QUT managers and staff to meet the legislative requirements a number of proformas have been developed for various risk assessment processes. A range of other Risk Assessment forms/systems are presented here for comparison.)
Chemical Risk Assessment
These chemical risk assessments have been developed by the areas listed and are used to help evaluate the risks and control measures for the handling and storage of chemicals in the workplace.
- Life Sciences FLAME model and guidelines (.doc, 508kb)
- This is a simplified risk assessment (doc, 107kb) for an individual substance. Use the more detailed risk assessment (Life Sciences FLAME) when using multiple substances and/or complex procedures.
Biohazard Risk Assessment
The biohazard risk assessments have been developed by the areas listed and are used to help evaluate the risks and control measures required for work involving biohazards.
Biohazard risk assessment (.doc, 546kb)
Field Work / Excursion Risk Assessment
The field work risk assessments have been developed by the areas listed and are used to help evaluate the risks and control measures for field work or excursions.
Manual Handling Risk Assessment
Manual handling injuries account for a significant proportion of all injuries at QUT. The use of a manual handling risk assessment will significantly improve the identification and mitigation of situations which may lead to manual handling injuries. Training is required to assess manual handling tasks.

- Plant
- First Aid risk assessment tool (provided by the Faculty of Health)*
- General risk assessment tool (provided by the Faculty of Health)*
*There are 2 First Aid risk assessments and 2 general risk assessments - the 1st is a Word template document for electronic completion and the 2nd is a PDF for print and hand completion.
