Lead Auditor

• Lead Auditor’s National Health and Safety Audit Tool – General
• Modelled on the National Self Insurer OHS Audit Tool 2007 and Queensland’s ‘Performance Criteria and Guidelines for Workplace Health and Safety Reports for Self-Insurers’
• All risk profiles discovered during the workshops are included in the tool.

• Excel spreadsheet which allows for the input of data collected by audit team from management and employee interviews. ‘No’ responses are entered into the spreadsheet so that the data can be used in the areas for improvement within the Audit Report. Percentages are calculated of persons that didn’t know or were unsure of the topic.
• Safety Question Collation Matrix
• A question is asked within the management and employee questionnaires regarding the individual’s perception of safety in their workplace. The appropriate answers are entered into the Questionnaire Collation Matrix, however the total numbers can then be entered into this excel spreadsheet where a graph can be produced and utilised within the audit report.
• Health and Safety Management System Annual Compliance Report
• Summary of Audit Report to be submitted to QUT’s Health and Safety Advisory Services

  Risk profiles for each faculty have been identified and appropriate tools have been developed, however there is also a General tool which incorporates all of the risk profiles, which can be used to modify tools at later stages in the audit program if required.
